What Does a Virtual Medical Scribe Actually Do?
A practical breakdown of what a virtual medical scribe does in a real practice - from real-time SOAP notes to inbox triage and chart prep.
A virtual medical scribe is a trained documentation specialist who joins patient encounters live - over secure audio or video - and produces clinical documentation in real time. That's the short answer. The reality of what a virtual medical scribe actually does in a working practice is broader and more useful than the description suggests.
Real-time SOAP notes
During the encounter, the scribe documents the chief complaint, history, exam findings as the physician verbalizes them, assessment, and plan. By the time the physician walks out of the room, the note is 90% complete in the EHR - awaiting only a quick review and signature.
Inbox and message triage
Between encounters, scribes triage portal messages, route refill requests to the appropriate physician, and draft replies for review. This eliminates a significant chunk of the after-hours inbox.
Chart prep before encounters
Before each appointment, the scribe reviews the chart, summarizes recent labs and visit history, and surfaces anything the physician needs to address. Physicians walk into rooms already informed.
Order entry and referrals
Scribes enter orders, queue up referrals, and pre-populate patient education handouts for the physician's review. None of these are clinical decisions - they're administrative steps the physician would otherwise have to perform after the visit.
What a scribe does not do
Scribes do not practice medicine, make clinical decisions, or sign anything. Every order, referral, and note is reviewed and signed by the physician. The scribe handles the administrative and documentation labor; the physician makes the clinical calls.
